Is American Express an elitist credit/charge card?

Small business people, view American Express as an expensive card to process. This cost consciousness limits where it is accepted. The correlation is that it only "rich people" can afford to own one because of it's limited acceptance. Is American Express, really an elitist credit/charge card?
