I can't agree with Pete Postlethwaite, since it's not clear which country he's supposed to be from anyway.

I thought Shannon Elizabeth's accent in "American Pie" was pretty terrible.

Kevin Costner as Robin Hood was the worst ever. Nothing else comes close.

I've just watched the new movie of Nicholas Nickleby. Great cast except the lead who was CRAP! So crap I've even forgotten his name, but his "English" accent was diabolical - nowhere near English; in fact nowhere near anything - it was this kind of hybrid of nigerian/swedish. Why didn't they just get an English person to do it?!

Meryl Streep should be in there for her atrocious Australian accent in...that movie about Lindy Chamberlain. Damn, what was it called?

I still don't know why Pete Postlethwaite is up there. His accent was very interesting and seemed to fit his somewhat sinister and mysterious character well. And as ohlefty points out, his origin is indeterminate.

He was trying to do a Pakistani accent and failed.

Josh Hartnett in Blow Dry should've been number one. It was painful to watch his attempts. Half the time you couldn't even hear what he was saying, seemed he was trying to hide his attempts..

I definitely agree with Heather Graham in From Hell. One minute she was English of indeterminate region, the next roosterney and then she was attempting a thick Irish brogue that was dangerously close to the Lucky Charms guy.

According to Redfoot the Fence, Kobayashi is a "Limey". This is not, obviously, the accent he is going for in the film, though.

Keanu Reeves from Bram stokers, terrible, raised a chuckle though.
