Since when has honesty been considered discourtesy. The world could benefit from a lot more honesty.

Either that or he's just being sarcastic. Usually if someone's all "Yes, you look ENORMOUS. I'm suprised the ground hasn't collapsed from underneath you you're so huge." in resonse to a question like "Do I look fat?" they're just joking and saying that because they can.

If someone you love is clinically obese, do you not have a moral duty to help them overcome this life threatening (and certainly life-shortening) condition? Resolution of health problems starts with acceptance. If it helps a fat person realise they're fat to tell them abruptly, go for it. If someone's just carrying a couple of extra pounds though, leave it.

Well, if she asks, he should be honest...but if he's with her, he must not mind. He should never just say it to be mean.

Honesty is very important, but calling her fat is being an assh*le. The last girl I dated was significantly overweight, and whenever she brought it up, I just said "You don't have to be thin to be beautiful" which was what I honestly thought (and still do).

What would be the discourtesy? Telling the "white lie" or bruising someone's ego? And how is confidence involved at all?

Well, to be honest, if a person is fat enough for it to 1) Be obvious to anyone who looks, and 2) actually be a health problem. They probably know it well enough themselves, and frankly if they don't like hearing it, they shouldn't go around asking 'Do I look fat'.
